Appointments

We offer telephone triage appointments, please call the surgery from 8am onwards Monday to Friday and you will be added to the doctor's triage list.

If a face-to-face appointment is needed the doctor will arrange this following your telephone consultation with them.

We endeavour to enable 24-hour access for emergency appointments and 48 hours, for routine appointments.

Extended Hours

Coventry Navigation 1 is offering Extended access to all patients of Clay Lane Medical Practice. You can book an appointment with GPs, nurses and other allied health professionals outside of normal opening hours, which include evenings or at weekends.

Extended access is about giving you the convenience and choice to have your appointment at a time that better suits you or based on your needs. Extended access appointments are delivered through “hubs”. This may mean you need to travel to a different practice than your own if you decide to book an extended access appointment.

Coventry Navigation 1 will start extended hours clinics from June 2022. Regular Saturday clinics will be delivered from COCHC  and Clay Lane Health Centre. Specialist clinics for renal and heart failure consultant, Women’s health clinics on Saturday will also be delivered.

Please Note: All Extended access appointments are booked Via your GP practice.

Extended Access Clinic opening times:

• Monday till Friday: 18:30 till 20:00
• Saturday: 10:00 till 14:00
• Sunday: 10:00 till 13:00

Emergencies

A serious emergency will be dealt with at any time of the day or night, and we have 24-hour telephone cover.

Chaperone

This practice has a chaperone policy. For arrangements, please ask at reception. 

Interpretation

If you require an interpreter, please ask at reception, who will contact DA Languages and arrange an interpreter for you. Please  leave at least 48hours for this.

Cancellations

If you cannot attend an appointment for any reason, please inform us as soon as possible in order for us to give the slot to someone else.

Home Visits

It is normally always best for us to see a patient at the Medical Centre. This is because we will have full access to their medical records and better facilities for both examination and treatment of any problems identified.

However, we do understand that some patients are genuinely unable to make the short trip into see us. Therefore, we do offer home visits in some limited circumstances, normally where a patient is

• Terminally ill or,
• Housebound and cannot be mobilised

Although a traditional part of general practice, home visits are time consuming. Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. So please help us to help you and our other patients by visiting the surgery whenever possible.

• Transport – We cannot undertake home visits for reasons of convenience or lack of transport. We will be happy to provide you with details of local taxi firms. From experience, we are aware that relatives, neighbours or friends are often willing to help out. Our responsibility to you is to resolve the medical problem you have; your responsibility is to take all the reasonable steps you are able to, to enable us to do that.
• Please request visits before 11.00am whenever possible as this allows the Doctor to plan their day accordingly. Late requests often lead to disruption of the appointment system and excessive waiting times for others.
• A doctor/nurse will call you back on most occasions to assess your problem. This is to enable the doctor to prioritise visits.
• It may be that your problem can be dealt with by telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a hospital attendance. It also prepares the doctor to collect some information required as necessary for the visit.
• In the past, GPs were able to do routine follow up home visits. Sadly, pressures of time and more patients needing attention means this is usually no longer possible.

You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP or PAVS (Paramedic acute visiting service) will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed.

You can be visited at home by a community nurse if you are referred by your GP. You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.
